Pathogenesis of Hepatitis B Virus (HBV) - Mediated Liver Injury

چکیده

As a “stealth” virus, hepatitis B virus (HBV) is not directly cytopathic for infected hepatocytes. Hepatic injury is due primarily to the response of the body’s immune system to either acute or chronic infection with HBV. In general, two types of host’s responses occur to viral infection, i.e., innate immune response and adaptive immune response. Current evidence suggests that the innate immune response does not play an important role either in HBV clearance or in liver injury. In contrast, the adaptive immune response mediated by cytotoxic Tlymphocyte (CTL) cells is kinetically associated with viral clearance and liver injury. This observation suggests that the pathogenesis of HBV is closely related to the CTLmediated immune response. One important way in which CTL cells mediate viral clearance is to secrete serine protease granzymes such as granzyme A and granzyme B which lead to the apoptosis of infected cells. However, HBV replication can upregulate the expression of apoptosis inhibitors such as serine protease inhibitor Kazal, or SPIK, resulting in the resistance of the cells to CTL-mediated immune killing. The inability of the immune system to clear HBV-infected cells can lead to chronic hepatitis B and development of HBV-cirrhosis and hepatocellular carcinoma. [NAJMS. 2011;4(1):1-6.]

Can Viral Load Predict Liver Histology in Patients With Chronic Hepatitis B?

Introduction: Hepatitis B virus (HBV) infection is a global public health problem. Serum HBV DNA measurement is a non-invasive test that predicts the liver disease progression. The aim of this study was to determine the HBV DNA level and look for a relation between the HBV DNA level and liver histology. Methods: This was a cross sectional study on chronic...

Immune Pathogenesis of Hepatocellular Carcinoma

Hepatocellular carcinoma (HCC) is a common complication of chronic hepatitis B virus (HBV) infection. The pathogenetic mechanisms potentially responsible for HCC during chronic HBV infection are not well defined. This study demonstrates that chronic immune-mediated liver cell injury triggers the development of HCC in the absence of viral transactivation, insertional mutagenesis, and genotoxic c...

Detection of SENV Virus in Healthy, Hepatitis B- and Hepatitis C-Infected Individuals in Yazd Province, Iran

Background: SEN virus (SENV) is the latest virus proposed as a cause of unknown hepatitis cases. Among nine detected genotypes of the virus, genotypes D and H are more frequent in hepatitis cases of unknown origin. The aim of this study was to determine the frequency of SENV-D and SENV-H genotypes in the sera of healthy individuals and hepatitis B and C patients. Methods: Totally, 200 serum sam...

Immunobiology and pathogenesis of viral hepatitis.

Among the many viruses that are known to infect the human liver, hepatitis B virus (HBV) and hepatitis C virus (HCV) are unique because of their prodigious capacity to cause persistent infection, cirrhosis, and liver cancer. HBV and HCV are noncytopathic viruses and, thus, immunologically mediated events play an important role in the pathogenesis and outcome of these infections. The adaptive im...

Anti-platelet therapy in the prevention of hepatitis B virus-associated hepatocellular carcinoma.

Previous studies in mouse models of self-limited viral hepatitis showed that platelets contribute to acute liver damage by promoting the intrahepatic accumulation of virus-specific CD8 T cells and, secondarily, virus-non-specific inflammatory cells. Built on these observations, a recent preclinical study took advantage of a previously established hepatitis B virus (HBV) transgenic mouse model o...
